J pouch surgery. A schematic shows a surgically constructed J‑shaped ileal reservoir attached to the anal canal by a suture line. In anatomical hierarchy, the terminal ileum (small intestine) is folded to form an internal pouch within the lower gastrointestinal tract, which is then connected to the distal anorectum in the pelvis. In colorectal surgery and gastrointestinal physiology, this structure—an ileal pouch–anal anastomosis (IPAA)—functions as a fecal reservoir to preserve continence and enable defecation after colectomy.
