A schematic depicts an immunologically cold tumor microenvironment (TME) in pathology and immuno-oncology. A densely packed mass of malignant cells with prominent nuclei is surrounded by fibroblast-like stroma and extracellular matrix, forming a barrier. Sparse lymphocytes remain at the periphery, with minimal infiltration of cytotoxic T lymphocytes (CTLs) into the tumor nest. This cellular–tissue hierarchy illustrates immune exclusion, a state associated with resistance to checkpoint blockade therapies.
