Kan du forestille dig, hvor sv\u00e6rt det ville v\u00e6re at f\u00e5 hendes\/hans opm\u00e6rksomhed, ikke? Nu skifter vi ekspedienten ud med enzymerne og kunderne ud med stofferne, og ekspedientens opm\u00e6rksomhed er det enzymsted, der er til r\u00e5dighed.&nbsp;<\/p>\n\n\n\n<p>Jeg er p\u00e5 holdet \"giv op og g\u00e5\", men i enzymverdenen findes dette hold ikke, og alle stoffer \"venter p\u00e5 at blive behandlet\", og derfor st\u00e5r alle l\u00e6gemiddelstoffer og venter p\u00e5 det n\u00e6ste enzymsted, der er ledigt.&nbsp;<\/p>\n\n\n\n<p>I sidste ende kan den h\u00e6mmende virkning f\u00f8re til toksisk akkumulering p\u00e5 grund af stigningen i l\u00e6gemiddelkoncentrationen i patienternes blodplasma.<\/p>\n\n\n\n<p>Et eksempel p\u00e5 det kan v\u00e6re h\u00e6mning af UGT-enzymer, der medf\u00f8rer en nedsat udskillelse af l\u00e6gemidler, hvilket medf\u00f8rer ophobning i kroppen.<\/p>\n\n\n\n<p>For at h\u00e6mningsprocessen kan finde sted, skal et af de to stoffer v\u00e6re h\u00e6mmeren, der dominerer alle enzymstederne og g\u00f8r enzymstederne utilg\u00e6ngelige for det andet stof at binde.&nbsp;<\/p>\n\n\n\n<p>Det h\u00e6mmede stof f\u00e5r s\u00e5ledes nedsat sin absorptionshastighed til et meget lavt niveau, hvilket betyder, at stoffet \"sidder fast\" i blodplasmaet og \u00f8ger dets biotilg\u00e6ngelighed til toksiske niveauer.&nbsp;&nbsp;<\/p>\n\n\n\n<h3><strong>Enzym-induktion&nbsp;<\/strong><\/h3>\n\n\n\n<p>P\u00e5 den anden side sker der det modsatte i enzyminduktionsprocessen. Det ene l\u00e6gemiddel accelererer enzymets evne til at absorbere det andet l\u00e6gemiddel, hvilket \u00f8ger absorptionshastigheden, og dermed mindskes l\u00e6gemidlets biotilg\u00e6ngelighed i blodplasma. Det kan f\u00e5 l\u00e6gemidlet til at miste sin terapeutiske virkning.&nbsp;<\/p>\n\n\n\n<p>Nogle l\u00e6gemidler har to eller flere metaboliseringsveje, hvilket betyder, at to eller flere enzymer sandsynligvis vil interagere med l\u00e6gemidlet. Det betyder dog ikke, at l\u00e6gemiddelinteraktioner ikke stadig kan forekomme, n\u00e5r to eller flere l\u00e6gemidler tages sammen.&nbsp;<\/p>\n\n\n\n<p>Og ud over tilstedev\u00e6relsen af anden medicin kan l\u00e6gemiddeloptagelsen ogs\u00e5 \u00e6ndres af genetiske variationer, sygdomme, alder og k\u00f8n.&nbsp;<\/p>\n\n\n\n<h2><strong>Cannabis-baserede l\u00e6gemidler tilg\u00e6ngelige p\u00e5 markedet<\/strong><\/h2>\n\n\n\n<p>I dag kan vi sige, at der findes to hovedtyper af cannabisbaseret medicin p\u00e5 markedet. Den, der indeholder alle plantens bestanddele, s\u00e5som cannabinoider, flavonoider, terpener og andre forbindelser, klassificeres som <strong>Fuldt spektrum,<\/strong> mens den anden type kun best\u00e5r af \u00e9n isoleret komponent, som CBD.&nbsp;<\/p>\n\n\n\n<p>Selvf\u00f8lgelig er der variationer imellem, som dem, der best\u00e5r af CBD: THC i specifikke forhold, og andre med CBD, der ikke er helt isoleret fra andre plantekomponenter.&nbsp;<\/p>\n\n\n\n<p>CBD er det hyppigste m\u00e5l i cannabinoidstudier, s\u00e5 derfor er det den cannabinoid, der er flest oplysninger om.&nbsp;<\/p>\n\n\n\n<p>Et stort antal offentliggjorte unders\u00f8gelser rapporterer om patienters forbedring, n\u00e5r de uds\u00e6ttes for CBD-behandlinger, mens en lille br\u00f8kdel af unders\u00f8gelserne h\u00e6vder, at der ikke er afg\u00f8rende resultater, og en lille del rapporterer endda om patientens forv\u00e6rring.&nbsp;<\/p>\n\n\n\n<p>Nogle af de symptomer og tilstande, der forbedres af CBD-behandlinger, er anfald, smerter, kr\u00e6ft, bet\u00e6ndelse, angst, neurodegeneration, multipel sklerose, depression og mange flere.&nbsp;<\/p>\n\n\n\n<h2><strong>P450-isoformer relateret til cannabinoider<\/strong><\/h2>\n\n\n\n<p>Det er allerede kendt, at CBD-metabolisme udf\u00f8res af to store enzymer fra P450-familien, isoformerne CYP3A4 og CYP2C19.&nbsp;<\/p>\n\n\n\n<p>Som ethvert andet l\u00e6gemiddel er biotilg\u00e6ngeligheden af CBD ogs\u00e5 modtagelig for \u00e6ndringer, n\u00e5r det uds\u00e6ttes for h\u00e6mmere eller inducerende l\u00e6gemidler.&nbsp;<\/p>\n\n\n\n<p>Derudover metaboliseres CBD ogs\u00e5, men i langt mindre grad, af isoformerne 3A4, 2C9, 2C19, 1A2, 2C8, 2B6 og 2E1.&nbsp;<\/p>\n\n\n\n<p>Som s\u00e5dan kan CBD fungere som en h\u00e6mmer og\/eller h\u00e6mmet. Nogle af de enzymer, der rapporteres at blive h\u00e6mmet af CBD, er CYP2C19, CYP2D6, CYP2C9 og CYP2C8.&nbsp;<\/p>\n\n\n\n<p>I UGT-familien kan CBD h\u00e6mme enzymerne UGT1A9 og UGT2B7. H\u00f8je doser af CBD kan have den modsatte effekt og blive angstfremkaldende, hvilket \u00f8ger angstniveauet.&nbsp;<\/p>\n\n\n\n<p>S\u00e5 det er vigtigt at finde den rigtige dosis til hver patients behov for at opn\u00e5 de bedste terapeutiske virkninger og udforske potentialet i, hvad planten kan tilbyde uden alvorlige bivirkninger.&nbsp;<\/p>\n\n\n\n<p>Et forkert valg af dosis kan f\u00f8re til bivirkninger som sedation, s\u00f8vnighed, diarr\u00e9 og nedsat appetit.&nbsp;<\/p>\n\n\n\n<p>Med hensyn til bivirkninger har en unders\u00f8gelse offentliggjort af Clinical Medicine Journal beskrevet en liste over alle bivirkninger observeret hos en gruppe patienter og andre detaljer fra kliniske fors\u00f8g.
