{"id":12274,"date":"2021-02-10T09:13:00","date_gmt":"2021-02-10T11:13:00","guid":{"rendered":"https:\/\/mindthegraph.com\/blog\/?p=12274"},"modified":"2023-01-05T13:46:34","modified_gmt":"2023-01-05T16:46:34","slug":"cannabis-and-body-pain","status":"publish","type":"post","link":"https:\/\/mindthegraph.com\/blog\/da\/cannabis-og-kropssmerter\/","title":{"rendered":"Cannabis og smerter i kroppen"},"content":{"rendered":"<p>I det seneste \u00e5rti har mange kliniske fors\u00f8g rapporteret om positive og lovende resultater i brugen af cannabisbaserede l\u00e6gemidler til behandling af neuropatiske smerter. <\/p>\n\n\n\n<p>Smertelindring i forbindelse med diabetes, kemoterapi, multipel sklerose, fibromyalgi, slidgigt og flere andre er blevet behandlet med cannabis. <\/p>\n\n\n\n<p>Tilsyneladende er der en forbindelse mellem eksogene cannabinoider og opioidreceptorer, hvilket resulterer i en stigning i patientens analgesi - den manglende evne til at f\u00f8le smerte. <\/p>\n\n\n\n<p>Patienter, der behandles med opioider sammen med cannabisbaserede l\u00e6gemidler, udviser en medicinsk synergieffekt, hvor opioideffekten \u00f8ges af tetrahydrocannabinol - THC, det st\u00f8rste psykoaktive molekyle i cannabisplanten. <\/p>\n\n\n\n<p>Forskerne ser positivt p\u00e5 disse resultater, fordi det er muligt at reducere doseringen af opioidmedicin og dermed forbedre livskvaliteten for patienter, der lider af smerter.<\/p>\n\n\n\n<p>Det endocannabinoide system er ligesom andre metaboliske veje ikke et isoleret system i den menneskelige krop, det kan p\u00e5virke og blive p\u00e5virket af andre signalveje, en af disse er tilsyneladende relateret til opioider og dets receptorer. <\/p>\n\n\n\n<p class=\"has-large-font-size\">Opioidreceptorer har ud over funktionen at signalere smertef\u00f8lelse ogs\u00e5 den rolle at modulere nogle kropsfunktioner som termisk regulering, hormonel stabilitet, analgetisk aktivitet, blandt andre.&nbsp;<\/p>\n\n\n\n<p>B\u00e5de cannabinoid- og opioidreceptorer er medlemmer af den G-proteinkoblede familie, som er proteiner, der har evnen til at p\u00e5virke neurotransmissioner, der er ansvarlige for smerteforplantning i hjernen og rygmarven.&nbsp;<\/p>\n\n\n\n<p>Behandlingen af patienter med kroniske smerter involverer normalt gradvist stigende doser af opioider, hvilket ofte f\u00f8rer til u\u00f8nskede bivirkninger som sedation, vejrtr\u00e6kningsforstyrrelser og forstoppelse. <\/p>\n\n\n\n<p>Efter et stykke tid begynder patienterne at udvise tolerance og resistens over for opioiddoseringen og opn\u00e5r ikke de terapeutiske virkninger, hvilket tvinger l\u00e6gerne til at \u00f8ge doseringen gentagne gange for at opn\u00e5 det samme niveau af smertelindring.<\/p>\n\n\n\n<p>En h\u00f8j dosis THC alene betragtes som smertestillende, men efterf\u00f8lges normalt af u\u00f8nskede bivirkninger som angst, hovedpine, mundt\u00f8rhed, svimmelhed, eufori og takykardi, mens lave doser slet ikke giver smertestillende effekt. <\/p>\n\n\n\n<p>Men n\u00e5r det kombineres med opioider, kan denne effekt \u00e6ndres fuldst\u00e6ndigt.<\/p>\n\n\n\n<p>Unders\u00f8gelser i dyremodeller har vist, at eksogene cannabinoider som THC kan \u00f8ge virkningen af opioidmedicin som morfin. <\/p>\n\n\n\n<p class=\"has-large-font-size\">THC's smertestillende virkning ser ud til at blive formidlet gennem opioidreceptorer, der har en krydsinteraktion mellem cannabinoidernes og opioidernes signalveje.&nbsp;<\/p>\n\n\n\n<p>Cannabisforskere rapporterer, at THC-administration kan stimulere produktionen af endogene opioidpeptider i kroppen, som ender med at binde sig til allerede kendte opioidreceptorer - delta og kappa - hvilket formidler cannabinoideffekten. <\/p>\n\n\n\n<p>En tredje opioidreceptor, mu, aktiveres ogs\u00e5, n\u00e5r der gives morfin. Aktiveringen af disse tre opioidreceptorer kan v\u00e6re forklaringen p\u00e5 den \u00f8gede effekt af den antinociceptive p\u00e5virkning - den analgesiproces, der blokerer for oplevelsen af en smertefuld stimulus - sammenlignet med resultaterne af behandlinger med morfin eller THC alene.<\/p>\n\n\n<div class=\"wp-block-image\">\n<figure class=\"aligncenter size-large\"><a href=\"https:\/\/mindthegraph.com\/blog\/wp-content\/uploads\/2021\/02\/opioids-and-cannabis.png\"><img decoding=\"async\" loading=\"lazy\" width=\"567\" height=\"803\" src=\"https:\/\/mindthegraph.com\/blog\/wp-content\/uploads\/2021\/02\/opioids-and-cannabis.png\" alt=\"\" class=\"wp-image-12275\" srcset=\"https:\/\/mindthegraph.com\/blog\/wp-content\/uploads\/2021\/02\/opioids-and-cannabis.png 567w, https:\/\/mindthegraph.com\/blog\/wp-content\/uploads\/2021\/02\/opioids-and-cannabis-212x300.png 212w\" sizes=\"(max-width: 567px) 100vw, 567px\" \/><\/a><\/figure><\/div>\n\n\n<p>A<a href=\"https:\/\/www.sciencedirect.com\/science\/article\/abs\/pii\/S0024320503010129?via%3Dihub\"> unders\u00f8gelse udgivet i 2004 af tidsskriftet Life Sciences<\/a> bekr\u00e6ftede forbindelsen mellem THC og morfin ved at h\u00e6mme opioidreceptoren ved hj\u00e6lp af specifikke molekyler. <\/p>\n\n\n\n<p class=\"has-large-font-size\">Tilstedev\u00e6relsen af blokerende molekyler resulterede i et fald i morfinens effektivitet og dermed dets terapeutiske virkninger. <\/p>\n\n\n\n<p>Dette resultat bekr\u00e6fter, hvor vigtig tilg\u00e6ngeligheden af disse receptorer og deres aktivering er for morfin- og THC-behandlingens effektivitet. Et andet eksperiment udf\u00f8rt med Naloxon - en opioidantagonist - blokerede THC-aktiviteten i forskellige omr\u00e5der af hjernen, som ventral tegmental area, hypothalamus og periaqueductal gray (PAG), hvilket tyder p\u00e5, at disse omr\u00e5der p\u00e5 en eller anden m\u00e5de er vigtige for interaktionen opioid-cannabinoid. <\/p>\n\n\n\n<p>Derudover rapporterer unders\u00f8gelsen ogs\u00e5, at musenes behandling med lave doser af THC og morfin sammen ikke f\u00f8rte til dosering af opioidtolerance.<\/p>\n\n\n\n<p>Denne unders\u00f8gelse og mange andre er blevet offentliggjort, og de underst\u00f8tter ideen om, at cannabinoider i kombination kan \u00f8ge opioidernes virkning. <\/p>\n\n\n\n<p class=\"has-large-font-size\">Cannabinoider kan tilsyneladende ogs\u00e5 \u00e6ndre tolerancen og afh\u00e6ngigheden af opioider. <\/p>\n\n\n\n<p>Langvarig brug af opioider er normalt forbundet med afh\u00e6ngighed hos patienterne, hvilket f\u00f8rer til misbrug af et stof, der oprindeligt kun havde et terapeutisk form\u00e5l. <\/p>\n\n\n\n<p>Derfor kan kombinationen cannabinoid-opioid - f.eks.
